What "Acute Onset" Coverage Means for Seniors
Virtually all visitor insurance plans exclude pre-existing conditions from routine coverage. But most include an important exception called acute onset coverage — and for seniors with chronic conditions, this is the most critical provision to understand.
Acute onset coverage means: if your chronic condition suddenly flares into a medical emergency — unpredictably and without warning — the plan covers that emergency treatment as if it were a new illness.
A 72-year-old with a 10-year history of diabetes who has a sudden hypoglycemic crisis requiring an ER visit? Covered. A 78-year-old with controlled hypertension who has a hypertensive emergency while sightseeing in Rome? Covered. Routine insulin, blood pressure medication, or scheduled cardiology visits? Not covered.
What IS Covered (Acute Onset)
- Sudden hypoglycemic or hyperglycemic crisis
- Unexpected cardiac event (heart attack, acute angina)
- Severe COPD exacerbation requiring emergency care
- Hypertensive crisis (dangerously high BP episode)
- Sudden severe asthma attack requiring ER treatment
- Acute stroke event
What is NOT Covered
- Routine prescription medications (insulin, blood pressure pills)
- Scheduled procedures for a known condition
- Ongoing management of a chronic condition
- Expected complications from a known illness
- Elective treatments or check-ups
Coverage by Age — What to Expect
Coverage options and pricing vary by age bracket. Here's what seniors with pre-existing conditions should expect at each stage.
Strong coverage, best rates
Seniors in their 60s have the most plan options and the most competitive pricing. Most standard visitor insurance plans accept travelers up to age 69 without restriction. Acute onset coverage is broadly available across all three of our carriers.
Full coverage still available
Coverage options remain strong through age 79. WorldTrips Atlas America covers through age 79. IMG Patriot International and GlobeHopper Senior both cover through age 99, with acute onset provisions intact. Expect a moderate premium increase over the 60s bracket.
Specialized senior-designed plans
IMG GlobeHopper Senior is purpose-built for this age bracket — covering travelers up to age 99. Trawick's Safe Travels Elite is available through age 89. Pre-existing condition acute onset is retained. This is where specialized senior coverage makes the biggest difference.
Coverage Guide by Condition
What you need to know about coverage for the most common senior health conditions.
Diabetes
Acute diabetic emergencies (hypoglycemic crisis, DKA onset) are covered under acute onset provisions. Routine insulin and glucose management are not covered — bring sufficient supplies.
Heart Disease
Sudden cardiac events — heart attack, acute angina, severe arrhythmia — are covered under acute onset. Elective cardiac procedures, routine check-ups, or medication refills are excluded.
High Blood Pressure
Hypertensive crisis (emergency-level blood pressure events) may be covered under acute onset. Routine BP management is excluded. Most seniors with controlled hypertension can find coverage without significant premium penalty.
COPD
Sudden severe COPD exacerbation requiring emergency care may qualify as acute onset. Routine COPD management, nebulizer treatments, and oxygen supplies are not covered.
Cancer (History)
Cancer in remission is a pre-existing condition. An unexpected new event or emergency complication may qualify for acute onset. Active cancer treatment is generally not covered.
Arthritis
Arthritis is one of the more travel-friendly conditions. Acute onset provisions may cover a sudden severe joint event requiring emergency care. Routine joint management is excluded.

Frequently Asked Questions
Can seniors with pre-existing conditions get travel insurance?
Yes. Most major visitor insurance plans available through Tower Hill cover acute onset of pre-existing conditions — meaning if a chronic condition like diabetes or heart disease suddenly flares up, emergency treatment is covered. IMG Patriot, WorldTrips Atlas America, and Trawick Safe Travels Elite all include acute onset coverage for seniors.
What does 'acute onset' mean for seniors with chronic conditions?
Acute onset coverage means that if your pre-existing condition causes a sudden, unexpected medical emergency during your trip — like a diabetic emergency, cardiac event, or severe COPD flare — the plan will cover that emergency treatment. It does NOT cover routine management of your condition, ongoing prescriptions, or expected complications from a known illness.
How much does travel insurance cost for a 70-year-old with diabetes?
A 70-year-old with diabetes traveling for 3 weeks typically pays $90–$160 for a comprehensive plan with acute onset coverage. Premium is primarily driven by age, trip length, and coverage level — not the specific condition. A 75-year-old with the same itinerary may pay $120–$200. Use our free quote tool for exact pricing.
Is heart disease covered under senior travel insurance?
Routine management of heart disease (medication, scheduled procedures) is not covered. However, if you have a sudden cardiac emergency during your trip — heart attack, acute angina, or sudden arrhythmia requiring emergency care — acute onset coverage applies. IMG Patriot International includes this, with up to $1M in medical benefits for travelers up to age 99.
What if I had a stroke in the past — can I still get coverage?
Yes, you can still purchase travel insurance. A history of stroke is a pre-existing condition. Should you have a new acute stroke event abroad, acute onset coverage would cover emergency treatment. Note that some plans exclude stroke as an acute onset condition for travelers over a certain age — review plan details carefully or call us at (832) 856-1704.
Does the look-back period affect seniors with long-term conditions?
Most plans use a 36-month look-back period. If you've been treated for, diagnosed with, or prescribed medication for a condition within 36 months of your coverage start date, that condition is considered pre-existing. For seniors with chronic conditions like diabetes, high blood pressure, or COPD, this almost always applies — which is why choosing a plan with strong acute onset provisions is essential.
Which plan is best for an 80-year-old with COPD?
IMG GlobeHopper Senior or IMG Patriot International are typically the best options for travelers 80+ with COPD. Both cover travelers up to age 99 and include acute onset provisions. GlobeHopper Senior is specifically designed for older travelers. Call us at (832) 856-1704 for a personalized recommendation.
Does Medicare cover seniors with pre-existing conditions traveling abroad?
No. Medicare does not provide coverage outside the United States, regardless of your condition or age. If you have diabetes, heart disease, or any other chronic condition and travel internationally, you need a separate travel medical insurance policy. This is one of the most important gaps for seniors to fill before any international trip.
