Stability Clause Requirement

Plans typically require AFib to be stable for 60–180 days: no cardioversion, no new antiarrhythmic medications, no emergency AFib-related hospitalizations, and no new ablation procedures. Ongoing anticoagulant therapy (warfarin, Eliquis, Xarelto) at a stable dose is permitted.
